Enrollment Procedure

To hold a spot for your child we will need a $50.00 registration fee.  You
can download the forms below or call us for a packet.  
The following items can be taken to our facility at

Enrollment application from Parent Handbook
Signed Parent Handbook receipt
Fully completed Emergency/Immunization Card  
A copy of your child's immunizations
Birth certificate

Employment Opportunities
Job Application
Fingerprint, TB tests, and CPR certification must
be completed upon hiring.
